Information and consent sheet for participants
You are invited to take part in this survey on the impact of health and safety capacity on firm value.
This project is being undertaken as part of the requirements for Chris Alderson's Masters Degree in Accountancy and Finance.
The purpose of this study is to examine if traditional financial concepts can be used to measure firm Health Safety and Wellbeing (HSW) capacity . The study will consider the effect of HSW disclosures on investors, directors and executives to encourage healthier and safer workplaces.
This study has been approved by the University of Otago Accountancy and Finance Department Ethics Committee. By starting the survey and signing the consent below you agree and have consented to the statements below:
By taking part in this survey, I understand that:
- I am free to request further information at any stage
- My participation in this project is entirely voluntary.
- I understand that participation is voluntary, and I can withdraw at any time without disadvantage to myself.
- There are no physical or psychological risks.
- Incentives will not be provided.
- The results of the project may be published but my anonymity will be preserved.
- Aggregated data only will be reported in any research publication
- I am above the age of 18 and currently live in New Zealand.
- This project involves an online survey. The questions will ask me about my views on the relationship between health and safety capacity and firm value. If I feel hesitant or uncomfortable about any survey questions, I may decline to answer any particular question(s) and/or may withdraw from the project without any disadvantage of any kind.
- The survey will take about 10-15 minutes to complete

Survey Instructions: This study investigates the role of health and safey capacity on percieved firm value . The survey will take approximately 10-15 minutes and you will be asked at the end if you would like to participate in a more detailed follow up interview with Chris Alderson.
